All content, including content generated with the assistance of artificial intelligence, is provided for general informational and organizational purposes only and does not constitute medical, nursing, pharmacy, nutritional, psychological, or legal advice. MeloCare does not practice medicine or any licensed profession, does not diagnose, treat, cure, mitigate, or prevent any disease, and does not exercise clinical judgment.

MeloCare does not provide doses, dosing schedules, titrations, or tapers, and does not instruct any user to start, stop, substitute, or change any medication, supplement, therapy, diet, or device. The prescription label and the treating care team control in all cases of conflict.

Scripture, prayers, and encouragement in MeloCare are drawn from a fixed library of public-domain translations selected by the congregation and are offered for encouragement only. They are not pastoral counseling, spiritual direction, mental-health treatment, or crisis intervention, and they are not a representation, guarantee, or promise of healing, recovery, or any other outcome. MeloCare is not a substitute for a pastor, chaplain, licensed counselor, or physician.

Use of MeloCare creates no physician-patient, pharmacist-patient, therapist-client, pastoral, fiduciary, or other professional relationship with MeloCare, with its personnel, or with the sponsoring congregation, its clergy, staff, or volunteers.

MeloCare is not an emergency, crisis, or monitoring service and is not reviewed in real time by any person. Posting a need does not summon help and does not notify emergency responders, clergy, or staff on any guaranteed timeline. In an emergency, call 911 or your local emergency number. If you are in crisis, call or text 988 in the United States.

Portions of MeloCare use large language models, which can produce incomplete, outdated, or incorrect output. Scripture references are rendered verbatim from the in-app library rather than generated, but all other generated content must be independently verified with a licensed professional before it is relied upon. MeloCare makes no warranty as to the accuracy, completeness, currency, or fitness for a particular purpose of any generated output.

Participation is sponsored by a congregation. An authorized church administrator approves each household, and the patient and the caregiver each provide informed, written consent before a workspace is created. Consent may be withdrawn by the patient or caregiver at any time, and the sponsoring congregation may end sponsorship at any time; in either case access ends and the household's locally stored information remains on their own device until they clear it. No person is enrolled automatically, and enrollment is not a condition of receiving care, ministry, or membership.

A sponsoring congregation and its administrators can see only participation status, the support requests a household chooses to post (such as a meal, ride, visit, or prayer), whether those requests have been claimed, and whether a household has become inactive. They cannot see diagnoses, medications, supplements, prognosis, clinician notes, private caregiver notes, or the content of a household's briefings, prayers, or conversations, and MeloCare does not store that information on its servers.

Information a household voluntarily posts to a shared support board is visible to the volunteers who can access that board. Households should post only what they are willing for those volunteers to read. Churches are not covered entities under HIPAA in this context, and this disclaimer does not create HIPAA obligations for a congregation.

MeloCare coordinates offers of practical help. It does not screen, background-check, train, supervise, license, insure, or employ any volunteer, and it does not verify that any task claimed is performed or performed competently. Volunteers do not provide skilled nursing, personal medical care, medication administration, or professional caregiving services through MeloCare. The congregation and the household are solely responsible for volunteer vetting, supervision, insurance, transportation safety, child and vulnerable-adult protection policies, and applicable state requirements.

Care profiles, medications, notes, and conversations are stored on the device they were entered on. MeloCare maintains no patient account, patient identifier, or centralized repository of health information, and cannot recover a household's information from a lost device unless the household saved its own encrypted backup. Each household is responsible for the security of its devices and of anything it prints or shares.

MeloCare is offered only to individuals at least 18 years of age and located in the United States, acting for themselves or as an authorized caregiver or personal representative of the patient. It is not offered in the European Economic Area, the United Kingdom, or Switzerland.

To the fullest extent permitted by applicable law, MeloCare is provided “as is” and “as available,” without warranties of any kind, express, implied, statutory, or otherwise, including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement.

To the fullest extent permitted by applicable law, MeloCare, its owners, personnel, and suppliers shall not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of data, loss of profits, personal injury, or medical outcome, arising out of or relating to the use of or inability to use MeloCare or to any act or omission of a volunteer or congregation, whether based in contract, tort, strict liability, or otherwise. Some jurisdictions do not allow certain exclusions, and in those jurisdictions liability is limited to the minimum extent permitted by law.

Each user must affirmatively accept this disclaimer before using the application; a record of that acceptance is stored on the user's own device.
